CONSENT TO PERSONAL DATA PROCESSING

In compliance with the provisions of Regulation (EU) 2016/679 on the protection of natural persons with regard to the processing of personal data (hereinafter referred to as the “GDPR”), I hereby give my consent to the above-stated company (hereinafter referred to as the “Controller”) to process personal data (hereinafter referred to as the “personal data”) provided by me, specifically:
